Our team of care providers assists during the hospital stay, helping seniors and their families plan for the transition and establish expectations.
We also offer a discharge planning guide called "Ready-Set-Go Home" to help care recipients plan ahead and have constructive conversations with their doctors. Our top priority is the health and happiness of our seniors and their families, and we are dedicated to providing them with the best home care and assistance.
After the senior has been discharged from the hospital and is back home, our team will provide a caregiver to help with their daily routine and ensure that the doctor's instructions are followed during the recovery care process. Our in-home services may include assistance with personal care, meal preparation, running errands, providing emotional support, and reminding the seniors to take their medication. In addition, our office staff will work hard to manage the care process and provide any necessary support.
