Ready-Set-Go Home is Visiting Angels Knoxville's innovative transitional care program that aims to reduce the number of times older individuals need to return to the hospital. This program is designed to offer older individuals full care as they transition from a hospital stay, with the goal of enhancing their well-being. To ensure the senior has a good transition back home, the Ready-Set-Go Home method includes both help while in the hospital and individualized care once they get home.
Why Hospital Readmissions Occur
Many older people experience a decline in their functional capacities following a hospital admission. It usually takes longer than expected for people to heal, despite their expectations of getting back to their regular lives as soon as possible. During this time of recovery, many seniors experience difficulties performing routine daily activities as a result of health issues or injuries. Their quality of life, freedom, and potential avoidance of hospital readmission are all negatively impacted by this.
There needs to be more extensive support for seniors after hospitalization since up to 20% of Medicare patients return to the hospital within one month of discharge. We created our Ready-Set-Go Home program to fill this void and offer the vital support that elders require for their home care services at this crucial moment.
How After-Hospital Care Helps

Our caregivers at home will meet with the senior's loved ones to discuss their needs and create a personalized strategy for in-home assistance and continuity of care. For a less stressful transition back home, you'll want to depend on this plan that considers the senior's unique preferences in addition to their expected care requirements.
During the recovery care phase, our Visiting Angels Knoxville staff can provide individualized post-acute care at home. Typical services this form of care provides include support with moving about, help with washing or showering, and meal planning and preparation. Our caregivers also encourage the elderly to take their medication as prescribed and keep track of their follow-up medical visits.
