We refer to this program as Ready-Set-Go Home because it paves the way for a comfortable transition from hospital to home and a successful recovery.

What Causes Hospital Readmissions?

Have you heard that nearly one in five Medicare beneficiaries are readmitted to the hospital shortly after their discharge? In fact, this statistic applies to the one-month window following a hospital stay. This period of time can be particularly difficult for elderly adults as they recover from illness or injury and the effects of hospitalization. During this time, seniors may be more susceptible to strain and may encounter mobility limitations or a lack of energy. As a result, they may be less likely to engage in the routines prescribed by their doctor and could even forget to take their medications properly.

How Our Transitional Care Program Helps Seniors in Milwaukee

Ready-Set-Go Home helps seniors in a number of ways. As we take a three-prong approach to care, we aim to cover all of seniors' needs prior to their discharge and as they get back to normal life at home.

For example, we begin with in-hospital care visits to help families prepare for the discharge and ensure they are supported as they have important conversations with physicians. Next, we turn to helping seniors once they've been discharged. Our caregivers can assist with a variety of daily tasks, from bathing and grooming to preparing healthy meals and keeping note of medication schedules. Seniors can also look to our caregivers for emotional support and companionship, which is often much needed during this challenging life stage.

Along the way, our office staff work behind the scenes to support the care process. They can facilitate deliveries and reminders, coordinate care schedules, and identify high-risk clients to promote their ultimate recovery.
