As a result, they may be less likely to engage in the routines prescribed by their doctor and could even forget to take their medications properly.

For example, we begin with in-hospital care visits to help families prepare for the discharge and ensure they are supported as they have important conversations with physicians. Next, we turn to helping seniors once they've been discharged. Our caregivers can assist with a variety of daily tasks, from bathing and grooming to preparing healthy meals and keeping note of medication schedules. Seniors can also look to our caregivers for emotional support and companionship, which is often much needed during this challenging life stage.
Along the way, our office staff work behind the scenes to support the care process. They can facilitate deliveries and reminders, coordinate care schedules, and identify high-risk clients to promote their ultimate recovery.
