Ready-Set-Go Home with Transitional Care

What Causes Hospital Readmissions?

Hospitalizations can be stressful and exhausting, both for elderly adults and the unpaid family caregivers who support them. Furthermore, many seniors undergo a decline in their functionality after being at the hospital. This can reduce their level of independence as well as their quality of life. As a result, some of the most basic tasks of everyday living can feel overwhelming for seniors, complicating their ability to care for themselves when they are at their most vulnerable. Unfortunately, the outcome of this situation is often a repeat visit to the hospital due to otherwise avoidable complications.

How Our Transitional Care Program Can Help Seniors in Pocatello

1. Care at the hospital. We know seniors have the best results when they receive support early on in their transition from hospital to home.
