"My wife was first diagnosed with dementia in 2015 and also diagnosed with Parkinson's disease a short time later. All of the Visiting Angels representatives who came to our home to provide care for us were wonderful. I appreciated their extra loving care and special attention to her many needs."

About Our Ready-Set-Go Home Service

After a hospital stay, many elderly people find that their functional capacities begin to diminish. While people want to return to normal lives as quickly as possible, the healing process frequently takes longer. Because of health problems or injuries, many seniors have trouble carrying out their daily tasks while recovering. Their independence, quality of life, and chances of not having to go back to the hospital are all diminished because of this.

Up to 20% of Medicare patients go back to the hospital within a month of leaving, so there needs to be significantly more help for seniors when they get out of the hospital. In response to this need, we developed the Ready-Set-Go Home program to provide the essential assistance that seniors need for their home care services right now.

Reasons for Readmission

Some of the most common causes of readmissions to the hospital are the following:

  • Being exhausted. Sometimes, older adults just don't have the stamina to take care of themselves following a hospital visit.
  • Memory problems. Doctors' orders can be lengthy and detailed, making it difficult for older people to remember everything.
  • Physical restrictions. Many necessary tasks, such as cooking and bathing, can be neglected when getting up and moving about, which is challenging for those recovering.
  • No help at home. The health of older people tends to decline when they lack a trustworthy caretaker.

How After Hospital Care Can Help Recovery

We offer a variety of services that our caregivers at home can help with, which include the following:

  • Palliative Care
  • Companion Care
  • Personal Care
  • Dementia Care
  • Social Care
  • Alzheimer's Care

The Importance of the Client-Caregiver Relationship

Building Trust

Having a solid client-caregiver relationship is essential to everyone involved. Our caregivers tend to become friends and confidants as well, so it's important that they're matched well with the client. We carefully match each person with the appropriate team member and consider personalities and needs. Our caregivers undergo extensive training to ensure they can provide the right assistance.

We encourage you to fill out this online form or call us at (803) 610-4200 if you're ready to begin planning your loved one's post-hospital care. To make sure our program is a suitable fit for your senior, we'll put you in touch with a coordinator who will ask you some questions.
