What is Senior Wellness Management?
Our Wellness Management program gives each client a tailored plan that will address their unique needs, helping families feel confident about their loved one's health. We focus on fostering a trusted relationship long before the need for basic care services are needed, offering a supportive resource for seniors and families that they can turn to at any time on the aging journey.
Medication Set-Up: A RN or LPN will make sure medications are properly organized and easily accessible for each senior. This helps to prevent missed doses, medication mix-ups, and ensures refills are obtained when needed.
Regular Nurse Visits: A Visiting Angels nurses will visit on a weekly or bi-weekly basis. This visit gives families peace of mind and helps to catch potential health concerns early promoting timely intervention when necessary.
Vital Sign Checks: At each visit, the nurse will take a full set of vital signs at each visit, monitoring blood pressure, heart rate, and other health indicators to monitor for any significant changes in their health and address concerns before they escalate.
Ongoing Communication with Your Care Team: Communication is at the heart of our Wellness Management program. We work in collaboration with families and medical professionals, keeping everyone informed about health updates and providing seamless support.
