What is the VA Community Care Network?
Visiting Angels of Granbury, which is a part of the Veterans Affairs Community Care Network, is able to assist you in covering expenses involved with at-home services for elderly veterans. If your loved one can't get the care they need from their local VA facility, the next best thing is to work with a provider in the Community Care Network.
Here's how veterans can be eligible to work with providers in the Community Care Network:
- To arrange a Community Care Network Home Evaluation for a loved one, get in touch with their primary care physician or a social worker via the VA;
- If your family member is qualified to receive services from the Community Care Network, their primary care physician or social worker will designate a VA case manager to conduct a Care Needs Evaluation;
- Your loved one may choose Visiting Angels of Granbury as their care provider once their case manager has assessed their needs and determine that in-home care is the best solution.
