What is Wellness Management?

Our Wellness Management program offers a services to support seniors in

 managing their health and wellness while remaining independent:

• Medication Set-Up: A RN or LPN will organize and manage your medications to be sure they are taken as prescribed and on time. This oversight will help reduce the risk of missed doses or accidental medication errors.

• Weekly or Bi-Weekly Nurse Visits: One of our nurses will visit you in person on a weekly or bi-weekly schedule to check on your overall health. These consistent visits offer families peace of mind because their loved one is getting a professional "check-in" on a regular basis.

• Vital Sign Checks: Our nurse will take a full set of vital signs at each visit, monitoring blood pressure, heart rate, and other key indicators to catch any health concerns or changes in condition.

• Communication with Your Care Team: We support a collaborative approach to senior care. We work closely with your healthcare providers—your physician, pharmacist, and other care team members—to make sure everyone is informed about any changes in condition, medication refill needs, and any other concerns.
