We start by conducting a consultation with the family and care recipient while they are still in the hospital. This gives us a chance to talk about the discharge plan, expectations for care assistance and specific individual needs with your family and medical team (if necessary). We will then create a personalized care plan based on the discharge instructions and make sure a caregiver is ready to go as soon as your loved one returns home.
