Senior Wellness Management

• Medication Set-Up: At each visit a nurse will set-up and check that medications are organized and safely accessible, helping seniors follow their medication regimes correctly.
• Weekly or Bi-Weekly Nurse Visits: Our experienced nurses will visit either weekly or bi-weekly to assess the senior's overall health and well-being.
• Full Vital Sign Checks: During the nurse visit, we will do a full set of vital signs, including blood pressure, heart rate and other indicators to track any health changes.
• Communication with the Care Team: We collaborate with each senior's physician, pharmacy and other care team members to provide care that keeps everyone informed and updated on concerns and changes in condition and helps to avoid running out of important medications.
